COVID-19: Requesting proof of vaccination

If you need urgent written confirmation of your COVID-19 vaccination you can request a letter from the Ministry of Health. They can only provide proof of vaccinations administered in New Zealand. Digital vaccination certificates will be available in November through My Covid Record.

Digital vaccination certificates coming late November

If you’re aged 16 or over, from the end of November you will be able to download digital vaccination certificates for use in New Zealand and overseas through My Covid Record.

If you need proof of vaccination before the end of November, you can request a vaccination confirmation letter – this takes up to 10 working days. 

Under 16s

You can’t currently use My Covid Record if you are aged 12 to 15.

If you’re aged 12 to 15 and require proof of vaccination (for example, for overseas travel) you can request a confirmation letter.


Vaccination confirmation letters

If you need proof of vaccination before the end of November, or someone aged 12 to 16 need proof of vaccination, you can request a letter from the Ministry of Health which confirms your vaccination status – this takes up to 10 working days. 

A vaccination letter from the Ministry of Health will include:

  • your name
  • your date of birth
  • your NHI number
  • the name of the vaccine you had
  • the batch number
  • the number of doses you’ve received and dates administered.

Request a vaccination confirmation letter 

The letter includes the personal information we hold about your vaccination in the COVID Immunisation Register (CIR), and your personal details which are linked to your National Health Index (NHI) number.
